Health Care, Life, and Disability Benefits

The Company provides qualified regular, full-time employees with a benefit package that includes medical, dental, vision, short-term disability and life insurance. Details of all insurance benefit offerings may be obtained from the Benefits Administrator in Human Resources. During approved leaves of absence and at termination employees may be eligible to continue coverage through COBRA.

Union employee benefits are separate as they are negotiated between Teamsters Local 853 and Sysco San Francisco and are administered through Teamsters Security Fund.

Medical Care

You have the option to choose from either a Preferred Provider Organization (PPO) or Health Maintenance Organization (HMO) to protect you and your family’s health and financial security.

Dental Care

You have the option to choose from the premium or basic option or waive dental coverage. The Preferred Dental Program offers freedom to choose any dentist at any time with discounted fees when you visit a participating dentist.

Vision Care

You have the option to receive vision care that may include: one eye examination and either one set of lenses or one set of contacts every year and frames are covered once every other year.

Health Care Enrollment

New employees are encouraged to make health care enrollment decisions as soon as possible. Eligibility begins within 60-90 days of employment. Open enrollment period is held October thru November. These changes take effect on January 1 of the following year. If you do not enroll during open enrollment, you will not be able to change your coverage during the plan year unless you have a “qualified change in status”. If you have a qualified change in status (marriage, divorce, birth, loss of spouse benefits), you are required to notify the Company of the change within 30 days of the change. Please see your Human Resources Representative regarding possible OPT-OUT options.

Flexible Spending Accounts (FSA)

Under the Flexible Spending Accounts (FSA), you may elect to set aside an amount to pay for certain eligible health and dependent care expenses on a before-tax basis. In this type of account, you can elect to defer a specific amount each pay period and the IRS does not consider the amount of the deferral as part of your taxable income. You have the option of contributing to the Health Care Reimbursement Account and/or the Dependent Care Reimbursement Account. These accounts are separate.
